2.  The 0pposing parent’s work address
3.  A list of addresses where the child/ren resided during the previous five years
4.  An outline of the custody arrangement you would like to see imposed
5.   The name and address of any other person(s) who exercise custody with the minor child(ren)
6. The child/ren’s full name(s) and birth date(s)
7.  The child/ren’s social security number(s)
8.  The child/ren’s most recent report cards
9.  A list of potential witnesses and their contact information
10.  Copies of any prior court orders or legal documents relating to the child(ren)
